Job Purpose

The Lead Continuum of Care (Co) Liaison will assist the local CoCs with the coordination intake and referral process for the Veteran’s registry. They will ensure that services are coordinated in such a way as to meet the greatest needs of the client first and in all areas for which the individual qualifies.

Duties and Responsibilities

  • Establish rapport with CoCs and convey important program information
  • Introduce comprehensive screening and assessment tools to the CoCs
  • Maintain electronic files per CoC, track meetings and events
  • Receive and process participant referrals from other CoC partners as needed
  • Refer participants as needed
  • Continue to follow up with homeless clients to offer permanent housing options
  • Attend meetings to provide CoC partners with updates on homeless veteran housing status
  • Other duties as assigned or requested

Qualifications

  • High school diploma or GED equivalent required
  • Associate’s degree preferred
  • Minimum of one year of experience in performing the above duties and responsibilities
  • Strong data entry skills
  • Experience working with Veterans is preferred 
  • Experience delivering services to SUD, COD, or homeless population is also preferred

Working Conditions

General office environment. The working environment is generally favorable. Lighting and temperature are adequate and there are no hazardous or unpleasant conditions. Work is performed in an office environment with standard office equipment available. Travel will be less than 10% and is mostly local.

Physical Requirements

Work is generally sedentary in nature and will require standing and walking for up to 20% of the time. There will occasionally be bending, lifting, and carrying of up to 10-15 pounds.
