The suicide prevention program is working towards reducing the risk of Veteran suicide.
Our program is centered on improving the Veteran’s well-being and baseline mental health status while
providing social support and connection to VA and community-based resources.

Eligibility
SERVED IN THE ACTIVE MILITARY
Discharged or released under conditions other than dishonorable.
SCREENING
Mental health and well-being screening to verify some risk factors for suicide
FOUND TO BE AT RISK
Be evaluated and found to be at risk of suicide using the Columbia-Suicide Severity Rating Scale (C-SSRS)
Also Available to Family of Veteran
- Over the age of 18
- Only eligible to begin services after an eligible Veteran is enrolled and engaged in the program

Referrals to community resources,
activities, and peer support specialists
EDUCATION
Providing educational material on recognizing
suicidal thinking and lethal means safety
EMERGENT NEEDS ASSISTANCE
Connection to services for transportation,
income support, child care, and more
