PSH Case Manager Position

Reports to: Program Manager

 

Job Purpose

The overall purpose of the Case Manager is to provide comprehensive case management, financial counseling, and assistance to help veterans and their families maintain housing, increase skills and or income and become more self-sufficient.

Duties and Responsibilities

  1. Delivery of case management and supportive services to homeless veterans; maintain case files
  2. Review Intake Specialist’s screen, matrix, categorization, and comprehensive assessment
  3. Make a determination of supportive service needs
  4. Develop a Housing Identification and Stability Plan
  5. Provide housing stability counseling:
    1. Awareness and referral to rental and rent subsidy programs
    2. Awareness and referral to housing assistance programs
  6. Educate participants on supportive service availability and client rights, including:
    1. Fair Housing
    2. Landlord-tenant laws
    3. Lease terms
    4. Rent delinquency
    5. Resolution or intervention of mortgage delinquency
    6. Home maintenance and financial management
  7. Develop and monitor Single Coordinated Care Plan for participants
  8. Make program referrals and establish linkages with appropriate agencies/service providers to assist participants to obtain needed supportive services, including, but not limited to: 
    1. Non-compensation benefits from the VA
    2. Vocational and Rehabilitation Counseling
    3. Employment and Training Services
    4. Educational Assistance
    5. Healthcare Services
    6. Childcare Providers and Assistance
    7. Protective Payee Services
    8. Legal Services
    9. Daily Living Services
    10. Direct Financial Assistance
    11. Emergency Assistance
  9. Execute participation agreements with participants
  10. Maintains accurate and timely records of clients through detailed case noting of assessments, contacts, and progress using required case management forms and maintains required HMIS records
  11. Coordinate with additional staff to provide pick-up/drop-off transportation services and administer bus tickets
  12. Assist participant in developing a personal budget
  13. Create a discharge plan for participants including referral options
  14. Collect rents
  15. Complete housing habitability inspections
  16. Other duties as requested or assigned

  • Intimate knowledge of veteran recovery community and issues
  • Must be flexible, maintain composure in difficult situations, and a sense of humor
  • Ability to relate to and interact with individuals from diverse cultural backgrounds
  • Maintain confidentiality of written and verbal information

Qualifications

  • Bachelor’s degree in social services
  • Minimum two years of experience in case management, life skills as well as conducting assessment and counseling services
  • Experience working with Veterans is preferred 
  • Experience delivering services to SUD, COD, or homeless population

Working Conditions

General office environment. The working environment is generally favorable. Lighting and temperature are adequate and there are no hazardous or unpleasant conditions. Work is performed in an office environment with standard office equipment available. Travel will be about 30% and mostly local. Time will be spent at client residences.

Physical Requirements

Work is generally sedentary in nature and will require standing and walking for up to 20% of the time. There will occasionally be bending, lifting, and carrying of up to 10-15 pounds.
